How to Make a Subject Access Request

To make a request under the Data Protection Act 2000 a Subject Access Request Application has to be made to the Council.

To make a subject access request you need to:

  • make the request in writing (using the forms below)
  • provide suitable proof of identity
  • provide sufficient details to locate the information you are requesting
  • pay a fee of £10 (Mid Beds District Council is entitled to charge a maximum of £10 for a subject access request)

You can appoint someone to act as an agent to make the request on your behalf.  You will need to complete the authorisation of agent form and provide proof of their identity.

Please return the completed form(s), enclose the fee (cheques payable to Mid Beds District Council), proof of identity (birth certificate, driving licence etc this will be returned to you) and return to:
